In 2006 two pediatricians, Henk-Jan Aanstoot and Henk Veeze, founded the Diabeter Clinic in Rotterdam, the Netherlands, because they wanted the freedom to innovate and transform how care was delivered to persons living with diabetes. When opening the first clinic, Henk-Jan and Henk’s vision of a “Healthy life with Diabetes” became a reality. Since then, Diabeter’s innovative value-based healthcare model has demonstrated its impact on persons with diabetes outcomes and efficiency, which was recognized with the World Economic Forum and VBHC Award in 2017 & 2023.

VCare: Our Disease Management System
Diabeter developed a disease management system VCare, collecting, measuring and analysing baseline data collection and outcomes in a standardized way whether patients are using multiple daily injections (MDIs), an insulin pump, or a sensor augmented pump (SAP). VCare measures more than 150 outcome parameters every night helping patients, HCP’s and management to be transparent, innovative and aimed at value-addition.
CloudCare: Our Population Management Solution
CloudCare®️ is a CE-marked population health management solution with remote monitoring function, advanced clinical triage service that integrates into existing workflows. Our solution enables efficient in-person and remote patient care, resulting in increased patient satisfaction, while maintaining excellent glycemic control. In short, it enables an efficient, more personalized, data driven care model.
How It Works
Patients can upload data from their glucose monitors, insulin pumps, and other devices, allowing healthcare providers to access real-time information on their blood sugar levels, insulin usage, and other vital metrics. CloudCare®️ uses advanced algorithms to analyze patient data and provide personalized insights and recommendations tailored to each individual’s unique needs and circumstances. CloudCare®️ identifies persons with diabetes in need of remote consultations or urgent in-person visits. Conversely, when a person with diabetes is deemed stable, follow-up visits can be arranged remotely or even canceled when deemed suitable.
